Physical Therapist - Adult Home Care | Per diem
Location: Serving Chittenden & Grand Isle Counties, VT Make a Difference at Home
Are you a passionate Physical Therapist looking for a rewarding career in home health? Join UVM Health's Adult Home Care team and help patients maintain independence, improve mobility, and enhance overall well-being-all in the comfort of their own homes. We are looking for therapists that are available weekdays, no weekends available at this tie. Must have prior PT experience; home health experience preferred but not required!
As a home health physical therapist, you'll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff , providing comprehensive, patient-centered care.Why Join Us?

As an Adult Home Care Physical Therapist, you will:
Perform comprehensive evaluations and assessments
Develop individualized treatment plans tailored to each patient's needs
Deliver skilled therapy interventions to improve mobility, strength, and function
Support patients in remaining in their homes safely and independently
Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties. Per diem support all of the 4 territories.Minimum Qualifications
Bachelor's degree or higher in Physical Therapy
2+ years of general PT experience, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
Home health experience preferred, but not required, we can train the right person!
Current Vermont Physical Therapist license, or willingness to obtain oneWhy You'll Love This Role
