Job Description
OverviewThe Deputy Director of Care Transformation sits within the Agency of Human Services (AHS) Office of Health Care Reform (HCR) and is responsible for supporting activities related to care transformation, including identifying, developing, and implementing policy and programmatic changes in response to Act 167 and Act 68 to ensure financial sustainability and adequate access across the health system in Vermont. A portion of the role will include overseeing Rural Health Transformation Program (RHTP) activities related to Care Transformation and related day-to-day operations, grants and contracts management, and coordinating with hospitals, providers, and state and federal partners.
This position reports to the Director of Care Transformation.
CORE RESPONSIBILITIES
Program Leadership
• Support the design and refinement of Vermont's Care Transformation Plan, integrating payment reform, delivery system redesign, and population health goals across statewide, regional, and hospital levels.
• Support development and scaling of innovative care delivery models, including alternative service models for rural and underserved areas such as telehealth, mobile care, and regional partnerships.
• Track progress toward program goals, including improvements in care access, workforce growth, and health outcomes.
• Use data to identify gaps in access, quality, cost, and equity; monitor key performance indicators; and support continuous improvement through evaluation of pilots and system reforms.
• Represent AHS in discussions with the federal government, the Legislature, and the public on Care Transformation progress.
Grants & Contracts
• Manage associated grants and multiple vendor contracts from award through closeout.
• Develop scopes of work, deliverable schedules, and compliance reporting.
• Ensure funds are spent appropriately and on time in accordance with federal requirements.
Coordination & Partnerships
• Coordinate across AHS departments and with other state agencies related to Care Transformation.
• Serve as a key point of contact for Vermont's hospitals, FQHCs, mental health agencies, and community partners.
• Engage providers, patients, and policymakers in co-designing transformation strategies and building consensus around resource allocation and service redesign.
• Facilitate governance and stakeholder advisory processes that guide program decisions.
Administrative Management
• Develop procedures and resolve administrative problems to keep work on track.
• Prepare reports and briefings for agency leadership, federal partners, and oversight bodies.
• Provide guidance and oversight to internal teams and external partners implementing transformation initiatives.
The Rural Health Transformation (RHT) project is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $195,053,740.44, with 100 percent funded by CMS/HHS.
