As part of the Department of Disabilities, Aging and Independent Living (DAIL) Adult Services Division, you'll work with the Money Follows the Person (MFP) grant assisting older adults, and individuals with disabilities to transition from institutional care to community living.  

 

As a Nurse Case Manager Utilization RN I, the successful candidate will suppport the goals of the MFP grant, working in a Transition Coordination role.  The MFP Transition Coordinator works collaboratively with participants, families, and community service providers such as hospitals, skilled nursing facilities, and case management agencies to connect individuals with services and supports.  Key responsibilities include program clinical eligibility assessments, participation in team-based care, and collaboration across the long-term care continuum.  Work is performed under the general supervision of the Money Follows the Person Program Director. 


 
Vermont RN licensure is required, knowledge of long-term care services and supports and experience with case management is preferred.


 
This position is grant funded through Money Follows the Person grant.  Work is performed in an office setting with some travel required.

Environmental Factors

Duties are performed in a facility, standard office setting and/or in the community. Extensive travel may be required, for which private means of transport must be available. Certain functions may require duty outside of normal work hours. Some resistance or objection to standards may be anticipated, requiring tact and diplomacy to achieve proper action. Duties involve need for effective communication and working relationships with a diverse group of stakeholders, including the individual, family members and a number of health care disciplines and service providers; and operational integration with various other health care reform initiatives. Critical assessment skills are necessary to promote personal and client safety.
