Job Description
RESPONSIBILITIES & SCOPE
Registered Nurse (RN) works as part of the AO team to promote a safe and healthy living environment for consumers in their own homes, or AO contracted or managed homes.
- Will perform physical health consultative support for individuals of Adult Outpatient.
- Implement and participate in the Individual Plan of Care/ Treatment Plan.
- Liaison with other community health service providers.
- Provide nursing consultation, assistance, training and delegation of medication administration procedures to staff of Adult Outpatient.
- Educate and counsel clients, family members, and primary care providers.
- Provide staff training to perform special care procedures.
- Participate in the development, and maintenance, of linkages with healthcare providers, advocacy for follow up for clients with complex health challenges, and health education for Adult Outpatient clients.
