General Description

The Aftercare & Alumni Coordinator will be responsible for facilitating the research of and admission to aftercare programs for DSRRP guests. The Coordinator begins to identify aftercare needs upon guest admission and engages in timely, efficient aftercare planning by working in close conjunction with the guest, family, and community resources to ensure safe discharge including transportation, housing and follow up appointments. The Coordinator also develops and supports our community of alumni through in-person and virtual meetings, activities and events. Additionally, the Coordinator works closely with Executive Director, Intake Coordinator and Outreach Coordinator to identify, grow and maintain relationships with behavioral health partners throughout the country.

Accountabilities

Aftercare Coordinator

  • Meet individually with guests on arrival and then at least weekly to process and formalize aftercare plan ideas.
  • Identify appropriate community resources and collaborate with guests, families, multi-disciplinary teams, and community agencies to achieve desired guest outcomes. Maintain contact with referral sources until appropriate resources are confirmed.
  • Facilitate weekly supportive aftercare group.
  • Ensure the guest understands post-discharge plans and arrangements for continuing care.
  • Coordinate guest transportation needs at time of discharge if appropriate
  • Coordinate aftercare with physician, therapist, peer support specialists, and other various individuals
  • Facilitate, collect, and aggregate client satisfaction survey data and collaborate with department leads to ensure aftercare needs are met and service expectations are exceeded.
  • Identify, develop and maintain relationships with community service agencies and residential facilities.
  • Data entry and management of Customer Relation Management (CRM) database.

Alumni Coordinator

  • Develops and maintains the alumni program
  • Introduces the alumni association to all guests from the moment of admission with the goal to grow meaningful, purposeful, and relevant involvement in the alumni association.
  • Ensure all guests pre-discharge have consented to communication from the alumni association.
  • Organizes in person and virtual onsite and offsite meetings, activities, and events for alumni, families, and friends in order to provide meaningful, purposeful, and relevant connections among alumni, support their long-term recovery, and continue connectivity between alumni and program
  • Facilitates well-being check calls and texts to discharged alumni on a prescribed schedule (30-, 60-, 90-day intervals).
  • Assists with the development and production of signature meetings and events (Alumni & Family Weekend, Weekly
  • Alumni Meetings, etc.) as well as develop new and enhance existing alumni and family traditions.
  • Assists with “alumni to guests” and “alumni to alumni” connections.
  • Attend relevant local community events, networking groups and continue communication / connection to local township, chamber of commerce, etc. on behalf of the Program.

 

Education, Experience, Skills

  • Bachelor’s degree in human services or other relevant field preferred.
  • Previous experience in discharge/aftercare planning, case management, or alumni relations within a SUD/recovery treatment facility
  • Ability to work collaboratively in a multidisciplinary team and build rapport with guests and alumni.
  • Experience utilizing Customer Relation Management (CRM) software and MS Office.


Work Conditions and Physical Requirements

  • Work Setting: Indoors, environmentally controlled
  • Body Positioning: Spend time sitting and standing, making repetitive motions, and spend time using hands to handle, control or feel objects, tools or controls. Frequently lift or move up to 10 lbs and occasionally lift or move up to 50 lbs
  • Communication: Contact with others, electronic mail, in-person discussions, public speaking, telephone, video
